KDA_4_60111A - --Various plastic food storage pans stored as clean per the PIC on the drying rack above the 3-vat sink have residual date mark stickers on the containers, one of which indicates a date of July 2015. COS: Equipment was re-washed. KDA_4_60111C - --The fan blade guards and the fan unit itself in the WIC have soil build up. KDA_4_60211E4 - --At the drive thru fountain drink machine the soda nozzle on the Pepsi fountain has syrup/soil build up. KDA_4_90311A - --In the back storage area a boxes of single-use cups and lids and a box of single use food wrap were stored directly on the floor. KDA_4_90311B - --Various stainless steel pans were stored on the drying rack stacked with liquid trapped between pans. KDA_5_501113 - --The outdoor waste dumpster has a missing lid. KDA_5_501115 - --The outdoor waste receptacle enclosure has a bicycle, construction cones, and debris in the enclosure. KDA_6_50111 - --Under the consumer fountain drink machine on the wall there is an exposed outlet protruding from the wall. The metal door frames of the door leading to the back kitchen area and the restrooms are rusted and corroding. The front service counter has exposed and warped wood. The tile half-wall of the front counter on the dining room side has missing tile that is covered by temporary vinyl sheets taped to the wall. There are broken and missing tiles and missing tile grout on the floor in the area of the cooking equipment. There are two holes in the wall where the door that opens to the back kitchen has damaged the wall.
