- KDA_4_10111A - --In the WIC a salad mix was stored in direct contact with a non-food grade Sterilite container. COS, VD.
- KDA_4_20211A2 - --There were 4 food storage container lids with large cracks in them making them difficult to clean. COS, VD.
- KDA_4_204112B - --There is no thermometer in the True RIC in the kitchen
- KDA_4_50111B - --The seal on the wood topped chest freezer is not fitting correctly.
- KDA_4_50113 - --There is a lot of dried food debris in the microwave.
- KDA_4_60111A - --Utensils stored in a container below the hot box in the kitchen had soot and dried food debris on them and in the container. COS, moved to be cleaned and sanitized.
- KDA_4_60111C - --The hood vents have grease accumulation on them. The sides of the grill and fryers have grease accumulation. The fans in the kitchen and by the bar have dust accumulation on them. The vent in the cheese dispenser and the inside of the RIF has soot buildup.
- KDA_4_60211E4 - --There is mold in the ice machine.
- KDA_6_20118 - --In the warewashing room the rafters and insulation are exposed.
- KDA_6_20216 - --The east end of the building there is a room where there are chest freezers, prep tables and packaged foods. The room is not enclosed with exterior walls to keep out weather or insects.
- KDA_6_30111 - --There was not a supply of hand soap at the handwashing sink in the kitchen. COS, soap supplied.
- KDA_6_30112 - --The handwashing sink in the kithen and the warewashing room did not have a supply of paper towels. COS, supplied.
- KDA_6_30114 - --There is not a sign to notify employees to wash their hands in the women's restroom or in the warewashing room. Stickers provided by inspector.
- KDA_6_50111 - --There is standing water in the bottom of the True RIC in the kitchen.
- KDA_6_501111A - --Behind the fryers there were 5 to 10 fresh rodent droppings on the floor. There were 20 to 30 fresh rodent droppings on top of the ware washing machine. In the ware washing room there were approximately 20 small flies in the garbage can. There were approximately 20 flies flying around the ware washing room.
- KDA_6_50112A - --There is food debris and water on the floor in the kitchen. There is water and food debris on the floor in the WIC. There are bottle caps on the floor below the below the 3-vat sink in the bar. There is black residue on the swinging door between the kitchen and the bar.
- KDA_7_10211 - --There was a spray bottle of purple liquid that was not labeled. PIC said it was degreaser. COS, labeled.
- KDA_7_20111A - --In the kitchen there was a bottle of sanitizer on a shelf next to a bag of tortilla chips. COS, moved.
