- KDA_3_30211A1b - --Boxes of raw bacon stored on shelf above one closed box of ready-to-eat honey ham, located in the meat department WIC. Removed the ham from that location.
- KDA_3_30511A1 - --Case boxes of food stored directly on floor in the storeroom Walk in Freezer at south wall. Cases of bananas stored on floor under the front retail isle display toward the front of the store. Bananas were elevated.
- KDA_3_50118A2 - --Small lug of open ham, for deli orders, with no date mark. According to meat manager this small lug was started in cutting from yesterday. Located in the meat department WIC. Marked the ham with proper date.
- KDA_3_50118A3 - --In-facility prepared, two-packages of devil eggs, packaged 6 per carton with a sell-by date of 10-14-15, located at the north retail open case and 6 packages of 1/2 pound slices of pastrami with sell by date of 10-13-15.These were located in the open case in front of the meat department. Discarded
- KDA_3_60211B4 - --No labeling information for name of manufacturer or address or location of this store. Removed from sale.
- KDA_3_60211B6 - --Four small individually wrapped caramel with walnuts, with no ingredients and nutrituion labeling. Only had the pricing bar code. Located in a discount pricing cart. Removed from sale.
- KDA_4_90311A - --Single serving plastic food containers stored in box sitting on the floor in front part of a slicer table in the meat department. This was elevated off the floor.
- KDA_5_20411A - --No hand sink conveniently located for employee hand washing in the back storeroom at the produce washing and handling station. Employees using the general public toilet rooms in the hallway that leads to the produce area.
- KDA_5_20511B - --A disconnected hose was being stored in the meat department hand wash sink bowl. Removed hose
- KDA_5_20515B - --The faucet at the meat department hand sink drips in a constant manner unless the supply value is turned off. Employees are having to turn on and off to prevent the drip. When turned on it does provide the ability to use the knee activated hot and cold water and deliver water for proper hand washing, but hands can get re-contaminated by turning off the value after the hand wash procedures are completed.
- KDA_6_20214 - --Employee and public use men's and women's toilet rooms opening into the hallway that separates the storeroom from the retail area, do not self close when not in use.
- KDA_6_20215A3 - --East, ground floor level, loading dock had a small 2 inch long by 1/2 inch high gap at the bottom of the small five foot wide overhead door. The gap in due to an open cavity of the floor cement at below level of the surface of the over-all concrete and can be easy patched filled to meet the bottom of the door. No evidence of rodent activity and they have monthly pest control. The two metal traps at both side of the door were empty.
- KDA_6_30111 - --Wall mounted Hand wash soap dispenser was empty at sink in the meat department. Employee refilled but would not dispenser soap and effort to clear the dispensing orifice of the discharge spout was not successful. Employee then provided a small pump soap bottle to this location.
- KDA_6_50118 - --Build up of soiled water dis-colors and remains at front edge of inside bowl since the bowl is un-level and does not allow some of the water to move to the drain.
- KDA_7_20111B - --One spray bottle of detergent cleaning liquid and two cans of insecticide spray on top of shelf stored above back-stock cans of food. Located in the back storeroom. Removed chemical from area.

North wall two-door RIFreezer food solid. Whole east wall, a bank of 30 RIFreezer doors- food solid. South wall: Left 16-ft open case raw meats 37-42, 8-ft center open case(hams,turkey,deli meats,deli salads at 41, 12-ft open case(lunch meats,hotdogs )41-43 range. Four-door south wall RIFreezer foods solid. South End of isle6 with a two-op-sliding chest freezer(pizzas,ribs,steak meats)foods-solid.
- KDA_KSA65_657b - --One 4-fl oz children's Dimetapp cold congestion medication Expired 6/15 and five-12.8 oz of powdered Enfamil Enfacare formula expired 1/Sept/15 and two quarts of Pedialyte liquid with use by date of 1/Jan/2015. Items were located out on the retail shelf. Manager removed from retail sale.
