- 3-304.15(A) - --Observed an employee with gloved hands rinse out a soiled pot in the 3 compartment sink. He then went to the kitchen area and pulled out the a ready to eat (RTE) tortilla with the same gloved hands without washing his hands and changing his gloves. COS Employee was educated and he washed his hands.
- KDA_2_30115 - --Observed 2 employee come inside the building and wash their hands in the 3 compartment sink. COS Employee was educated. They washed their hands at the handwashing sink.
- KDA_3_30212 - --Container filled with a oil, margarine, and water was not labeled. A container fill with pinto beans did not have a label.
- KDA_3_30511A1 - --Thawed fries and chicken tenders were stored on top of a container in front of the handwashing sink by the deep fryer. COS Cook was educated. Food items were put in freezer.
- KDA_3_30512 - --A container filled with taco shells was stored on top of reach in freezer and underneath electrical bug zapper. COS Cook removed shells and place on a shelf away from the bug zapper.
- KDA_3_50112 - --Fries 82F and chicken tenders 79F were sitting on a container in front of the handwashing sink in the kitchen area next to the deep fryer. Cook said it had been there for an hour. COS Food was put in freezer.
- KDA_3_50116A1 - --Observed the cooked mexican rice stored under a prep table in the kitchen near the hot and cold MT. The rice had a measured temperature of 82F. The cook said it had been there since 3pm. Three containers of mexican rice had measured temperatures of 141F, 139F, and 152F. COS The mexican rice was put on the flat stove top for rapid heating and hot holding.
- KDA_3_50118A1 - --In the WIC, tomato based sauce had the prepared date of 9/26 and 2 pans of tomato base salsa had the preparation date of 10/24. COS The food items was discarded.
- KDA_4_20211A2 - --Stored on a clean storage rack, plastic food contact containers had cracks on the inside and on the edge.
- KDA_4_60111A - --Stored as clean on a clean storage rack, were plastic food contact containers that had food debris on the interior surface as they were stacked. COS Containers were removed and placed in the warewashing area to be washed rinsed, and sanitized.
- KDA_4_60212B - --The white and stainless steel microwave had dried food debris on the interior walls and the ceiling.
- KDA_4_90111A - --Stored on a clean storage rack, metal and plastic food contact containers were stacked wet.
- KDA_5_20511B - --In the kitchen area, stored in the handwashing sink near the deep fryer was tongs. COS Tongs were removed.
- KDA_5_20515B - --The handwashing sink handles (cold & hot) at the bar does not turn allowing water to run. The handwashing sink at the wait staff area near the chips does not work. Employee said someone was supposed to come to fix it.
- KDA_6_20215A3 - --The back door in the kitchen area, had a 2 inch gap at the bottom of the door.
- KDA_6_30111 - --The handwashing sink in the wait staff area near the chips did not have soap.
- KDA_6_30112 - --The handwashing sink in the bar area did not have paper towels.
- KDA_6_50112A - --In the warewashing area on the floor is standing water.
- KDA_7_20111B - --Paint was stored above a "Tamarindo", a concentrated drink mixture in the back storage room by the ice maker. "Pine Sol" and other chemicals were stored above bottles of hot sauce stored in a box.

The cook said they had just came off the stove an hour ago and was cooling. I educated the cook and the instructed him to place the pans in the WIC for rapid cooling.

Employee said someone was supposed to come to fix it. PIC said he will call someone out today to fix the handwashing sinks.
