- KDA_3_30211A1b - --In walk in cooler, a container of raw bacon wrapped pork was setting above ready to eat (RTE) flour tortillas. COS - pork moved to lower shelf.
- KDA_3_50113A - --A package of raw beef was setting on food prep counter to thaw. Employee stated it had been there approximately 1 1/2 hours. COS - beef moved to walk in cooler.
- KDA_3_50115A - --In food prep area, 3 containers of green chili were setting on countertop to cool for 1 1/2 hours, per employee. Containers were 119F, 113F, and 128F. COS - employee placed containers in walk in cooler for rapid cooling.
- KDA_3_50118A1 - --In walk in cooler, containers of RTE, prepared on site, refried beans (dated 10/14/15), red chili (dated 10/13/15), and green chili (dated 10/14/15) where held over 7 days. Employee stated that the date mark was an old date mark. The preparation dates were: refried beans 10/21/15, red chili 10/21/15, and green chili 10/23/15. COS - items dated correctly.
- KDA_3_50118A2 - --In walk in cooler, a container of RTE polish sausages (opened from original package) did not have consume by date. Employee stated they were opened on 10/26/15 in the AM, over 24 hours. COS - container dated.
- KDA_4_204112A - --There are no thermometers available to read the ambient air temperature of the walk in cooler and reach in coolers holding potentially hazardous foods (PHF).
- KDA_4_50111A - --The metal shelves in walk in cooler are rusty and corroded.
- KDA_4_60111A - --In clean storage, a knife had dried food debris. In kitchen area, the can opener blade had dried food debris. Employee stated is was last used over 24 hours prior. COS - knife and can opener placed at 3-vat sink for cleaning.
- KDA_4_60111C - --There is a build up of dried mix/moisture inside both cappuccino machines. There is a build up of food debris on the metal shelves in walk in cooler. There is a build up of dust/lint on fan guards and ceiling of walk in cooler.
- KDA_5_20511B - --At hand sink, there were dirty dishes stacked in the sink. Corrected on site (COS) - dishes moved to 3-vat sink.
- KDA_7_20111B - --On shelf above food prep/cookie oven, there was a can of WD-40 and Flex Seal. COS - chemical moved to bottom shelf away from food. On shelf in back storage area, a can of Rustoleum was setting above single use cups. COS - can moved to another area for storage.
- KDA_Defacto - --Cold Holding: Walk in cooler - refried beans 41F. Reach in cooler - 41.3F ambient. Coke reach in cooler - 42.4F ambient. Pepsi reach in cooler - 42.9F ambient. Freezers ok. --Hot Holding: Hot display cabinet - red chili burrito 123F, bean and cheese burrito 108F, breakfast burrito 104F. Establishment uses TPHC, and had paperwork/logs showing all items had been in hot cabinet for approximately 2 hours.
