PIC said that noodles were cooked on 11/11, pork was cooked on 11/10, chicken was cooked in the morning on 11/12, beef was cooked in the morning on 11/12, generals chicken was cooked in the morning on 11/12, egg rolls were cooked on 11/09.
- KDA_3_50119A1a - --Facility was using TPHC for Generals Chicken, Sweet and Sour Chicken, Egg Rolls and Crab Rangoon but did not have written procedure in place.

- KDA_4_60111B - --The seals on the rice cookers between the lid and equipment had visible food debris and buildup accumulation.
- KDA_5_40211 - --Cut celery and cabbage were stored in a 3 Vat sink basin directly connected to the sewage system. PIC COS by removing and discarding food.
- KDA_6_30112 - --Handwashing sink in the kitchen did not have disposable towels or other hand drying provision available. PIC COS by supplying paper towels.
