- KDA_4_60213 - --There is an accumulation of water, debris, and possibly mold in the basket on top of the ice machine holding the 2 ice scoops.
- KDA_4_70211 - --The dish machine (stationary, single-rack, variable temperature) is not sanitizing wares. After being run once, these temperatures were recorded with a maximum-registering thermometer during cycles 2, 3, & 5, respectively: 149.7F, 154.2F, 154.6F. Further, the log sheet kept for this dish machine by employees reflects similar low "sanitizing rinse" temperatures for August 20th-27th, and the entire month of November, (with the high recorded of 165F). (No temperatures were recorded in September, & no log sheet can be found for October) No corrective actions were noted, or taken, according to the employee . COS - she set up a sanitizing compartment in the 3-compartment sink, with 200ppm Cl, and 124F water. Educated about reducing water temperature to between 55 and 75F, to prevent dissipation of chlorine. Started re-sanitizing all utensils and wares. Served students on foam food trays for this lunch service, and called for ASAP repairs on dish machine.
