Observation: The chest reach-in freezer in the smoke room has excessive amount of ice build up. Must defrost as often as needed. When frozen food bags are opened shall keep covered to prevent contamination.

Observation: Employee beverage with no cover or lid being stored on food contact surface in the bar. Food employee must drink from closed beverage container to prevent mouth to hand contamination. Removed and corrected on site.

Observation: There is leak on three compartment sink faucet in the kitchen. All equipment must be in good working order. Repair --Observation: A hose in mop sink is extended into basin of mop sink. Must provide an 1"air gap between hose and flood rim of mop sink. Keep the hose off the sink. Corrected on site.

Observation: Paint on the wall is peeling and chipping away between bar and mop room. It must be smooth easily cleanable and non absorbent,so physical facility is maintained in good repair. Repaint it.

Observation: No procedure for Norovirus clean up.
