Observation: Faucet on three compartment sink slight leaks. Repair to maintain in good condition.

Observation: 1) All condiment containers have no lid. 2) Didn't put the whole lid on ice bin at front. Must have lid to prevent sources of contamination.

Observation: Chlorine sanitizer bottle not labeled. Chemical must be identified with the common name after removed from original container.

Observation: Visible build up on can opener. Clean more frequently to prevent contamination.

Observation: Several boxes of single service items were stored on the floor in storage and at front service area. Single service items must be stored at least 6" off floor to prevent contamination.

Observation: Didn't provided a thermometer in Ture reach-in cooler in storage. Must have thermometer for each cooler unit to monitor temperature. Corrected at time of inspection.

Observation: Ice scoop stored on the top of water heater. Must provide a clean contact surface to store ice scoop. Removed. Corrected at time of inspection.

Observation: The facility uses QA sanitizer in three compartment sink. Must provide QA test strips to ensure concentration is maintained correctly.

Observation: Used mop was stored in mop bucket with water. Must put on hook to allow to air dry in between uses. Corrected at time of inspection. REPEAT VIOLATION
- Sanitization methods - hot water, chemical
Observation: Bleach water in spray bottle is tested too strong. The concentration of chlorine sanitizer must be maintained between 50-100ppm. Made a new sanitizer at time of inspection. Corrected.

Several powder containers were not labeled on the top of kitchen line cooler. Containers holding food which could be mistaken for other food must be identified with the common name of the food.

Handwashing signs not posted in both restrooms. Every hand sink to be used by employees must has sign posted reminding employees to wash their hand.
