Observation: The following coolers have torn door gaskets: True pizza cooler by register (both), "Frozen Food" freezer in retail (Left), and True cooler in retail (middle and right). Must replace to ensure adequate temp is maintained.

Observation: Pop nozzles in retail area have become soiled and must be cleaned.
- Food temperature measuring devices are provided and readily accessible
Observation: Deli express cooler in retail area does not have a thermometer inside. Must have a thermometer in all coolers that hold TCS (time/temp control for safety).
- Handwashing signage
Observation: Hand washing signs not posted in the back hand sink and the mens bathroom. Must post a hand washing sign at all hand sinks used by food service employees.
- Smoke free air act
Observation: Official no smoking sign not posted. Must post official no smoking sign at all public entrances.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: Observed raw eggs and bacon stored above ready to eat food in the milk cooler (by entrance). Must store all raw products below all ready to eat food to prevent contamination.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Milk cooler has become soiled and must be cleaned.
- Plumbing system maintained in good repair
Observation: 3 hole sink faucet has a leak. Must repair or replace to prevent buildup of bacteria.

Observation: The following areas are no longer smooth, easily cleanable, and non absorbent: FRP behind oven is coming off wall, Ceiling tiles above pop syrup rack soiled, FRP behind 3 hole sink missing, cove base missing on left side of 3 hole sink, floor tile broken by Whirlpool freezer, and floor tiles missing below pizza prep cooler (by register). All areas must be repaired or replaced to meet all above criteria.

Observation: Drawers in retail area must get cleaned as often as necessary to prevent build-up even if nothing is currently stored in them. Corrected on site.
- Plumbing system maintained in good repair
Observation: Leaking faucet for 3 compartment sink in kitchen area. Must replace to maintain in good working condition.
- Equipment location, installation, repair, and adjustment
Observation: Torn door gaskets on 1) Reach-in freezer door for where bagged ice is stored (currently is duct taped) and 2) Both doors on pizza preparation reach-in cooler. Repair to maintain in good working condition.
- PHF/TCS food maintained at 135°F or above, except during preparation, cooking, cooling, or when time us used as a public health control
Observation: Pizza in hot hold was 130*F. Must hot hold at least 135*F or above. Hot hold unit was turned up to 145*F. Use thermometer to ensure adequate hot holding temps. Corrected on site.
- Food storage - preventing contamination from the premises
Observation: Chip rack must be maintained at least 6 inches off the floor for easy cleaning and food protection.
- Cleaning of cooking and baking equipment
Observation: Clean out debris in ovens.
- Sanitization methods - hot water, chemical
Observation: Quat sanitizer buckets tested below 100ppm. Must change as often as necessary to ensure adequate concentration (200ppm). Corrected on site.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Must date mark all cooked meats, mashed potatoes, deli meats, and hot dogs if packages has been opened and not used within 24 hours. Have 7 days from date mark to use product or discard.
