Observation: No hand soap available at the hand washing sink by the walk in cooler. Employee supplied more soap.

Observation: Refried beans prepared 10/21 stored in the walk in cooler still had an internal temperature of 62F and were not properly cooler. Rice prepared 10/21 stored in the walk in cooler still had an internal temperature of 58F and was not properly cooler. Shredded chicken prepared 10/21 stored in the walk in cooler still had an internal temperature of 55F and was not properly cooler. All product discarded by the person in charge.

Observation: Hot water faucet at the three compartment sink is missing. Water can still be turned on and off.

Observation: Fresh pico de gallo(44F), verde sauce(48F) held in the prep line cooler were not able to maintain proper cold holding temperature. Person in charge stated the foods were placed in the cooler 3 hours earlier. Lids were placed on the tops of the cooler opening. The cooler temperature came back down to 41F. Refried beans (57F), ground beef (48F), ambient air (52F) in the walk in cooler are not able to maintain proper cold holding temperature. Person in charge was not aware how long the main cooler was out of temperature. A service call was made at time of inspection. Perishable foods in the main cooler were discarded by the person in charge. No perishable foods can be stored in the main cooler until properly holding at 41F or below.

Observation: Cooked rice (135F) was stored in a pan with a lid on the green shelf in the kitchen cooling. Person in charge removed the lid for rapid cooling. Then decided to add the rice to the steam table for hot holding.

Observation: Debris build up on the blade of the table mounted can opener. Person in charge cleaned and sanitized the blade.

Observation: Chlorine bleach sanitizer tested too strong >200 ppm. Person in charge mixed up a new batch at the correct concentration.

Observation: Employee cutting up fresh tomatoes for the pico did not first wash the tomatoes. Person in charge washed the tomatoes then resumed cutting.

Observation: Raw frozen chicken was thawing in a hot water bath in the three compartment sink. Person in charge removed the chicken and cook it on the grill.

Observation: Leak observed collecting into a bucket from the PVC pipe under the three compartment sink. Water drips from the three compartment sink after the water is turned off at the faucets.

Observation: Raw cubed steak stored above ready to eat foods in the line prep cooler. Raw shell eggs stored above ready to eat foods in the walk in cooler. Person in charge removed and stored the foods correctly.

Observation: Procedures for Norovirus bodily fluids clean up procedure is not available onsite. Inspector provided employee with a copy of a clean up procedure.
