Observation:Ceiling tiles and vent covers in the food prep areas covered with dust and debri that could potentially fall into food product. Floors in food prep and food storage areas excessively dirty under and around equipment and hard to reach areas.

Observation: Unshielded lightbulbs in the upstairs dry goods storage area where opened containers of food is stored.

Observation:Dirty wiping cloth stored on package of paper towels on back prep line.

Observation:Shelving unit on the back prep line where clean plates are stored has an excessive build up of dust/debri. Broken glass on shelf inside refrigerated chest where glasses are stored (bar). Inside of refrigerated chest cooler in the bar containing juice is dirty with food residue.

Observation:Soap dispenser at the bar handsink does not dispense soap.

Observed a buildup of dust and residue on ceiling tiles in the food prep area of the kitchen primarily around ventilation.
