Observation:The running water dipper well is not operating properly at the time of visit. Water is running, but is not staying in bowl and scoops are not free of food.

Observation:Walkways around facility and leading to entrance. Broken areas pose injury hazard. Additionally, parking lot has areas in need of repair that customers may need to walk over in order to get to sidewalks and building.

Observation:Make table on the line at far right is struggling. Food on top temping above 45 and up to 60F. Currently, ice is under pans due to repair issue with the unit. If unable to repair unit, then unit will need to be replaced or not used. Provided information to GM regarding using time as a public health control. Record system and documentation must be kept.

Observation:Detail cleaning is needed in bar area, under and around equipment. Discolored strawberries noted, build-up noted and area is wet. Also, routinely clean condenser units.

Observation: Thermocouple is very dirty and has hardened food debris on it.Ice machine has a bit of blackened growth in it, scoops that are in use need to be washed rinsed and sanitized at least once @ 24 hours. plumbing emergency is still not resolved and waste is pouring out. Main line out of the building is clogged, GM is working on a resolution. original company's equipment was broken. Contact has been made with DNR and the city regulators as well. A long term correction will need to be made as this has been a recurrent plroblem. outside of building, the plumb lines including storm sewer is the Mall's. This all connects into the City's lines.

Running water dipper well has soiled dippers in it and the water is not running. Pop nozzle holder has stagnant liquid and growth inside of it. Tube appears to be plugged. Staff cleaned and sanitized this at the time of the visit.

There are some issues with drains in the kitchen area. Mng. states that plumber is expected yet today for repair. Handwash sink in kitchen by dishwash area, floor drain in center kitchen area and also drain nearest to dessert prep area. Also, please discuss drain status of the food prep sink. Food prep sinks need ot have indirect drains. Additionally, at the bar area-the waste from ice bin drips directly to the floor and the waste line from pop nozzle appears to drip to floor. Both should be directed to the floor drain.
