Observation: Installed bakery, no hand sink provided.

Observation: Container of sanitizer solution stored directly next to open container of cherries. Broaster cleaner stored next to box of onions and over paper towels.

Observation: Baked goods prepared on site, does not have contact information on labeling.
- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation: Working container of green beans/bacon with an expiration date of 11/30/15 in prep table - disposed of.

Observation: Rhubarb ice cream topping not from an approved source - will no longer use and find an approved source.

Observation: Lettuce in top of prep table at 54.3 degrees. Sliced tomatoes at 45 degrees.

Observation: upon arrival, mechanical ware washing machine not sanitizing. After priming machine, began working properly. Establishment to test each morning.
