Basic - Fly sticky tape hanging over food/food preparation area/food-contact equipment. Observed hanging fly trap over 3 compartment sink and clean dishes. Owner said they will remove the trap because they just had an electrical bug machine installed.

Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Various items ( chicken, lamb, etc) prepared the previous day with one prepared back on Wednesday (2 days prior) with no date mark.
Intermediate - Spray bottle containing toxic substance not labeled. Observed blue substance hanging near handwash sink in back kitchen unlabeled as to its contents.
