  • Observed floor area(s) covered with standing water-dishwash area and 3 bin sink on opposite side of walk-in cooler.
  • Critical - Observed heavy black gooey encrusted material on can opener blade.
  • Critical - Observed mop/cleaning waste water dumped into slow draining mop basin.
  • Critical - Observed sanitizing solution exceeding the maximum concentration allowed. Chlorine in wiping bucket by kitchen restroom tested at 300+ ppm.
  • Observed single-service articles stored without protection from contamination - coffee/tea filters at wait station.
  • Critical - Observed uncovered food in holding unit/dry storage area. Black beans in walk-in cooler, bottom shelf, just inside door-not covered.
  • Critical - Observed unlabeled spray bottle. At least one with blue liquid on bar top.
  • Observed wall by back door into kitchen heavily soiled with accumulated black debris.
  • Waste line missing at soda gun holster-bar back at wait station.

  • Critical. Observed employee drink in a food preparation or other restricted area-Dishwashers drink was on cook/prep table. Corrected On Site.
  • Observed cutting board grooved/pitted and no longer cleanable-board at salad reach-in cooler.
  • Observed old labels stuck to food containers after cleaning.
  • Critical. Dishmachine chlorine sanitizer not at proper minimum strength. Dishwasher found at 0 ppm. Ran 3 times before testing at at least 50ppm. Corrected On Site.
  • Critical. Handwash sink not accessible for employee use at all times. Bar type mats in handwash sink at 3 bin sink.
  • Observed attached equipment, hood surfaces soiled with accumulated, runny, dripping, black grease dripping from flat surfaces other than filters, over grill/fryer areas.
  • Carbon dioxide/helium tanks not adequately secured.
  • Critical. Observed employees provided in-house training not performing their work duties in a manner consistent with the Food Code requirements. Dishwasher stated he was new and did not know about test strips and did not check sanitation level in machine. Corrected On Site.

  • Observed attached equipment soiled with accumulated dust. Exhaust fan in employee restroom heavily soiled.
  • Ceiling not smooth and easily cleanable. Both mens and womans restrooms as well as employee restroom in kitchen area.

  • Observed equipment in poor repair-old refrigerator, ice machine and flat top grill on premises. Owner/operator states they will be discarded.
  • Critical. No handwashing sign provided at a handsink used by food employees-at least one missing.
  • Ceiling not smooth and easily cleanable-employee restroom and storage areas.
  • Observed high grass and garbage in caged area at back door. Area could be a harbourage for pests.
