Last pest control service provided October 1,2014.

Risk Control and Pest Control Plans submitted.Owner was informed that future summary suspensions may result in revocatio of the establishment's business license.

Correct stated violations within 45 days.

Establishment in compliance.Delicatessen license restored and premises permitted to reopen for business.
