Sewage & waste water properly disposed Single toilet on site obstructed at call, summary suspension issued pending repair/replacement and re-inspection for compliance. Location must be approved by DOH before any food sales to the public is authorized.
Toilet facilities: properly constructed, supplied, & cleaned Single toilet facility for establishment not operational at call employees are leaving out of location and using facility next door. Repair or replace unit observed obstructed toilet;
Comments:
CFPM Ms. Paruin Firoz on duty at call, I contacted Ms. Firoz concerning report of inoperatble toilet room complaint and advised operator to close location until facility was repaired. Also informed her of the food code and related actions for operating without a toilet room on site. Operator failed to close and location was operational with customers at my arrival. I attempted to enter the tiolet room and staff member informed me the faclity was closed and signage observed posted on the door stating such. Summary suspension issued for operating with sewage back up and no toilet facilities.
