  • KDA_3_30211A1b - --In the dairy WIC a box of raw bacon was stored on a shelf over fully cooked ham. Also, a box of raw bacon was stored on a shelf over a box of bottles of orange juice. COS, both boxes of bacon were stored in an approved location.
  • KDA_3_30511A1 - --In the meat WIC boxes of gravy and other foods were stored directly on the floor. Also, in the filling station storage room a box of concentrated soda were stored directly on the floor.
  • KDA_3_50116A2 - --In the pizza MT the following items had the following temperatures. sliced tomatoes 44 F, cooked ground beef 46 F, cooked meatballs 44 F, cheese 46 F, fresh mozzarella 45 F, cooked italian sausage 45 F. At 9:30 the PIC said he put the items in the MT at 5:00 AM. Made on site pizzas in the retail, 3 tier, open air case ranged in temperature from 43 F to 51 F. The temperature was higher on the right side than the left side and higher on the bottom tier than the top tier. Pizzas that were stacked on top temped higher than the ones on the bottom. COS, Pizzas that temped 43 were kept but 6 large pizzas, 8 small pizzas, and 10 flat bread pizzas were discarded.
  • KDA_3_50118A1 - --In the deli meat case one open package each of deli ham, roast beef, and turkey were date marked 1/12. The computer system prints the discard date when the package is opened. COS, VD.
  • KDA_3_60211B1 - --In the bakery a spray bottle of clear liquid was not labeled. An employee said it was water. COS, labeled.
  • KDA_4_30111 - --The pizza MT and the open air case for the made on site pizzas are 1 unit. The unit is not holding PHF's at 41 F or below. The ambient temperature at the MT is 39 F. The ambient at the open air made on site pizza case is 41.7 F on the right side and 37.6 on the left side. The MT where the crocks for the pizza toppings are stored have a metal guard around them that has holes in it by design. The cold air is escaping through those holes.
  • KDA_4_60111A - --In the deli department there was dried food debris on the blade of the cheese slicer. The manager said the slicer is cleaned every 4 hours and at night when they close. The manager said the cleaning process is using a sanitizer spray to clean in place. COS, the slicer dismantled and cleaned and the manager educated. Also, crocks stored as clean for the pizza MT had stickers and residue on 50% of them. COS, moved to be cleaned and sanitized. Also, below the oven sheet pans are stored as clean. 65% of the pans have barcode stickers and residue on the bottom of them. COS, moved to be cleaned and sanitized. Also, a spatula stored as clean in the bakery area had dried food debris on it. COS, moved to be cleaned and sanitized. A knife stored as clean in a rack on the wall had food debris on the blade. COS, moved to be cleaned and sanitized.
  • KDA_4_60111C - --The meat slicer used to slice deli meats in the deli had food debris on the base of the slicer. COS, cleaned.
  • KDA_5_20315 - --The carbonator to the soda fountain can not be accessed to clarify if it has a dual check valve with an intermediate vent in the correct location.
  • KDA_5_20515B - --The hot water faucet on the filling station's 3-vat sink is leaking.
  • KDA_5_50117 - --The unisex toilet room at the filling station does not have a covered waste receptacle.
  • KDA_7_20111A - --In the produce area a spray bottle of sanitizer was hanging on a rack next to boxes of food service gloves. COS, moved.
  • KDA_7_20111B - --Also, a rack next to the produce area spray bottle sanitizer and other chemicals were stored over to boxes of food service gloves. COS, gloves moved to approved location. In the large warehouse 2 packages of face wash was stored over a box of french fried onions and other foods. Also in the large warehouse, a box of soap was stored over a box of single use food storage containers. Also, boxes of shaving cream were stored on a box of single use cups. In the small warehouse a box of fingernail polish was stored on a box of candy. COS, moved.
  • KDA_7_30111B - --On a retail clearance aisle near the front of the store containers of air freshener were stored on a shelf over beef jerky. Also, on a retail clearance shelf bottles of makeup and face wash were stored on a shelf over napkins. Also, on a retail shelf near the pharmacy, bottles of bubbles were stored on a shelf over single service plates. COS, moved.
