- KDA_3_50116A1 - --On the steam table red sauce was holding at 100F and enchilada sauce was holding at 101F. In the cheese dip crock, cheese dip was holding at 106F. The items had been cooked fresh and placed into the steam tables within the last few hours. COS-reheated. (repeat)
- KDA_3_50118A1 - --In the RIC jalapeno cream cheese, cheese dip, and cooked beef were dated with a preparation date of 8/24. In the make table, verde sauce was dated with a cook date of 8/24. A container of mashed potatoes were dated with a cook date of 8/17. The items were held greater than 7 days. COS-discarded. (new)
- KDA_3_50118A2 - --In the RIC a container of diced tomatoes, red sauce,rice and hot dogs lacked a date mark. The PIC said they were prepared or opened on Saturday. In the make table a pan of tamales lacked a date mark. The PIC said the tamales were cooked on Saturday. In the wait station RIC individual bowls of flan lacked date marking. The PIC said they were made on Friday. COS-items were dated. (new)
- KDA_4_60111A - --There was a dead moth on aluminum foil near where fried tortilla bowls are stored. There were maggots and moths on the inside and outside of containers used for storing rice, peppers and beans. COS-items were moved to be cleaned. (repeat)
- KDA_4_60111C - --Maggots were crawling on a shelf on a prep table in the kitchen.
- KDA_5_20511B - --Buckets were being stored in the kitchen hand sink. COS-buckets were moved.
- KDA_6_30111 - --There was no hand soap at the kitchen hand sink. COS-the cook placed dish soap at the sink. (repeat)
