Observation: The following food in bulk containers have no label: 1) Bulk food container containing white powder in dry storage area, 2) sugar in dry storage areal, and 3) little shaker with powder sugar has no label on it for identification.

Observation: The following equipment have food debris and require more frequent cleaning to avoid bacteria build -up: 1) inside top of microwave, This is REPEAT violation, 2) Food mixer had food debris on the top and around.

Observation: Quat sanitizer in two red buckets tested very low below 150 ppm. Manger made fresh one and corrected on site.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Sour cream and butter milk in walking cooler opened with no date marked. Manager stated they were opened yesterday he put the date marked and corrected on site.
- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation: 1) Ready to eat-vanilla ice cream dated (10/19) and 2) chocolate fudge cream dated(7/17) stored in plastic container that has been opened in dry storage area. Label instruction written to use 1 week after opening after that store in refrigerator or freezer. Manager voluntarily discarded and corrected on site.

Observation: There is a leak on thee compartment sink faucets in the kitchen.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: Ground beef stored above pork loin in walk in cooler. Manager removed and stored on lower shelf and corrected on site.

Observation: Found couple cracked plastic containers. All container and utensil must be maintained in good condition. Replace as needed.

Observation:Food establishment must have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment.
- Sanitization methods - hot water, chemical
Observation: Sanitizer was tested too weak. Must replace sanitizer more frequently.
- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation: All food employee must fill Form 1-B Conditional Employee or Food Employee Reporting Agreement.

Several plastic containers was observed wet after being stacked on the rack in kitchen. Must allow fully air dry before stacking. Corrected at time of inspection

Both containers of sugar and flour were not labeled. Container holding food which could mistaken for other food must be labeled with the common name of the food.

Cardboard box is reused as container for ice cream toppings in dry storage. Cardboard is not cleanable and nonabsorbent surface. Should not be reused for other items. Must provide easy cleanable container for this purpose use.

The sanitizer sink on 3 compartment sink is leaking. Repair. All plumbing must be in good repair.
