- Food storage - preventing contamination from the premises
Shelving with 2 liters is not 6" off the floor. All food and food items must be stored a minimum of 6" off the floor to aid in cleaning. Move bottom shelve up.

1) Ceiling has a hole from water damage. Repair as needed. Cauking at hand sink and wall juncture by dish machine must be redone. 2) Cove base under dish machine is falling off. Replace.

1) Grease build up in pizza oven hood. 2) Vents on reach-in freezers and make table has dust build-up. Clean areas and maintain cleaning at frequency to prevent future build up.

Unable to detect chlorine sanitizer in dish machine. Chlorine must be maintained between 50-100 ppm for proper sanitization. New bucket was put on and machine was primed. CORRECTED.
- Common name-working containers
Sidewalk salt was in bucket labeled sanitizer. All chemicals must be labeled with common name of product when removed from orginal container. CORRECTED.
