Observation: Food establishment must have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment.

Observation: All food employees must read and sign Food Employee Illnesses Reporting Agreement.

Observation:Must have at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service is a certified food manager.

Observation: Visible buildup on ice machine chute. drain ice, wash, rinse, and sanitize.

Observation: Both water handles on three compartment sink leak. Repair. All plumbing must be maintained in good working order.

Observation: Gasket on the pizza kitchen line cooler is torn. Replace as needed.
- Cleanability of floors, walls, and ceilings
Observation: 1) Provide caulking behind hand sink next to Coca cooler. 2) Repaint floor at dish machine area to provide cleanable, and non-absorbent surface.

Soap and bleach water be mixed together in the bucket for cleaning tables and food contact surfaces. Must have separate buckets for each chemical solution to prevent chemical reaction. Corrected at time of inspection

Observed several chemical buckets and bottles not labeled. Must label all working container with common name for easy identification. Corrected at time of inspection

1) Piece of bare wood behind mop sink. REPEAT VIOLATION. 2) Wood sufaces of front counter have paint chipping away. Must clean more frequently to prevent contamination.
- Light bulbs, protective shielding
Light bulbs were not shielded above buffet area. Light bulbs must be shielded, coated, or otherwise shatter-resistant in area where there is exposed food.

Raw eggs were stored above ready-to-eat food in Coca Cola reach-in cooler. Raw animal food must be stored below ready-to-eat food to prevent contamination. Corrected at time of inspection

Back door has light showing around outer edges. Must replace weater striping or door sweep to prevent entry pest.

Overall, the facility have good date marking. Observed a half gallon milk was not date marked in Coca Cola reach-in cooler. Must date marking all potentially hazardous food if not used within 24 hours after opened or preparation. Have 7 days from date marked to use or discard. Corrected at time of inspection
