KDA_4_20211A2 - --Repeat violation - one plastic food container and one plastic food container lid are cracked (both discarded). This prevents proper cleaning and pieces of plastic can end up in foods. KDA_4_60111C - --Repeat violation - The reach in freezer in the kitchen has mold and soil on the bottom door gasket and gasket area. KDA_6_501111A - --New violation - in the kitchen, one live roach was crawling up the wall near the grill. As per the kitchen manager, they have a hired pest control service that sprays for insects and sets out sticky traps. Pest control method observed was sticky traps.	12/07/2015	4	Follow-up	Out
KDA_3_50114A1 - --At 3:39 pm, in a reach in cooler, chili that was cooling was 99 degrees. The manager stated it went in to the cooler at about 2 pm. At 4:14 pm, the chili was 88 degrees. It did not cool down to 70 degrees or less within two hours (discarded). Also, the chili was in in a crock pot container with a lid, and was also in a home style cooler. Reviewed proper cooling procedures with the manager and suggested using one of two commercial coolers for cooling foods. KDA_3_50118A1 - --In a reach in cooler, open commercially prepared ready to eat brats not date marked. As per the manager, they were opened November 16th. They were not discarded within seven days (discarded). KDA_3_50118A2 - --In a make table, ready to eat lettuce cut yesterday and put in the cooler more than 24 hours ago and open commercially prepared ready to eat sausage opened yesterday and more than 24 hours ago not date marked. Dated both items (more than 24 hours ago as per manager). KDA_4_10119 - --In the make table, bare wood slats are being used as spacers and to hold food containers up. KDA_4_20211A2 - --One plastic food container is cracked, this prevents proper cleaning and pieces of plastic can end up in foods. KDA_4_204112B - --No thermometer in the reach in cooler in dry storage holding cheese. KDA_4_60111A - --One plastic measuring pitcher stored as clean has a sticker on the outside of the container (removed to 3-vat sink). KDA_4_60111C - --Food debris is on the inside of the make table in the bottom and west side. The reach in freezer in the kitchen has food debris on the inside in the bottom. On the bottom door gasket and gasket area are mold, soil, and food debris. KDA_4_90411A - --In the dining room, single use plastic forks, knives, and spoons are stored in a plastic cabinet and it is self serve for customers. The utensils are stored different directions. Must store these so the handles are up, which is impossible to do in a drawer. KDA_6_50111 - --In the kitchen just west of the fryers is a beam that is rotten at the bottom (and moldy). KDA_6_50112A - --Grease is on the floor and wall behind the grill. Debris and food debris is on the floor by the walls in the dry storage. There is a beam just west of the fryers that is moldy at the bottom. KDA_7_10211 - --A wiping cloth bucket with quaternary ammonia (tested positive) not labeled (labeled). KDA_7_20111A - --In dry storage, liquid hand soap stored next to and touching a stack of single use coffee filters in a plastic bag (moved). KDA_7_20111B - --In dry storage, one container of liquid hand soap and two containers of liquid pot and pan detergent stored above onions and soda syrup (moved). KDA_Defacto - --[quaternary ammonia] in a wiping cloth bucket = 200 parts per million --Facility had a consumer advisory poster, but with the name of the previous facility.
