Facility must have backflow prevention repaired before restarting dish machine. FWL issued per KDA District manager. Facility requested to send email once backflow prevention and dish machine is repaired.

  • KDA_3_50114D - --In kitchen, over grill, flat of raw eggs in shell 62. COS-employee said they had been out for 1 hour so he placed them in the refrigerator.
  • KDA_3_50116A2 - --In kitchen, over grill, carton of pasteurized eggs 67. COS-Employee said eggs were out for about 1 hour so employee placed carton in refrigerator.
  • KDA_4_20211A2 - --In kitchen, on clean dish rack 8 plastic containers stacked on shelf cracked and missing plastic. COS-Manager chose to remove plastic pans and discarded them.
  • KDA_4_60111A - --In kitchen, 10 plastic pans and 1 spatula stored as clean with adhesive label residue and dried on food on food contact surfaces. COS-manager removed pans and spatula to dish area for cleaning.
  • KDA_4_90311B - --In kitchen, on clean dish rack, plastic pans not allowed to dry before stacking together. COS-manager removed all containers from clean dish rack for re-cleaning and drying. In kitchen, by ice machine, container of clean ladles, and serving spoons not inverted or covered in storage. COS-manager removed them for correction in dish area.
