Critical: Food - Potentially Hazardous Food - Cold Holding* (repeated violation) Observation: The Meat and Cheese Make unit was not operating properly to cold hold all of the stored foods at 41 F or lower. Foods were cold holding between 36 F and 50 F
Correction: The Pizza Prep Make unit upper section was operating improperly cold holding foods between 49 F and 53 F.
Critical: Time as a Public Health Control* Observation: The Finishing Table cooler unit was not operating at proper temperature and time as a public health control (TPHC) was being used for the stored Sliced tomatoes without any written procedures for the use of TPHC with potentially hazardous foods.
Correction: Provide written procedures for the use of time as a public health control. The procedures must include the following steps or practices: a. All foods must be marked with the time within which they must be served or discarded. b. The foods are to be served or discarded within four hours from the time when the foods are removed from temperature control c. Foods in unmarked containers or packages, or for which the time expires are to be discarded and d. Written procedures are maintained in the food establishment and made available to the regulatory authority upon request e. Once time is implemented as a control measure for potentially hazardous food, no other measures may be substituted.

Re-inspection revealed the following: Both Make units were operating at ambient air temperature of 39F - 40F. However, cold held foods were @ 47F - 48F. Therefore, lower temperature of both Make units even further
Cooling, Heating, and Holding Capacities Observation: Both Make units were not cold holding potentially hazardous foods at temperatures equal to or lower than 41F.
Correction: Fix both cooler units so that they can cold hold foods at 41F or lower.

A re-inspection will be made in about 10 days to check on the cold holding units. Food Permit re-issued today.
Person in Charge Observation: Foodhandlers do not all have training in food sanitation nor do they have a foodhandler's card.
Correction: All foodhandlers are to obtain training in food sanitation in order to get a foodhandler's card.
Jewelry - Prohibition Observation: Foodhandlers were wearing jewelry on their arms and hands while preparing food.
Correction: Ensure employees are not wearing jewelry (with the exception of wedding bands) on their arms and hands while preparing food.
Utensils - In-Use - Between-Use Storage Observation: Dispensing ice scoops for the ice making machine were being stored inside a soiled and broken container.
Correction: Discard the broken container.Wash the ice scoops and store them on a clean surface or in a clean container.
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Sliced tomatoes (54F), cheese (47F) and cold cuts (51F) inside upper section of the large make units were cold holding at improper temperatures.
Correction: Sliced tomatoes were out since 3:30 PM and must be used up by or discarded within 4 hours (7:30 PM). Relocate other foods to a colder unit.Things to keep in mind: Do not stack food items too high
Equipment - Good Repair and Proper Adjustment Observation: The door gaskets for the meat cooler and W-in cooler were worn.
Correction: Replace the worn door gaskets.
Lighting, Intensity Observation: Shelving inside the W-in cooler were mold covered while surfaces close to the cooler fan were very dusty and were likely not noticed because the lighting inside the W-in cooler was dim and less than 10 foot candles.
Correction: Improve the lighting inside the W-in cooler so that areas that are in need of cleaning can be seen to clean. Provide at least 10 foot candles of light.
Physical Facilities - Cleaning Frequency and Restrictions Observation: Some drain covers were clogged and needs cleaning.
Correction: Clean all clogged floor drain covers.
