The purpose of today's visit was to conduct a risk factor assessment inspection. Observed facility clean and organized.

The purpose of today's visit was to conduct a routine inspection. Observed facility clean and organized during a busy preparation period. CFM on duty during today's inspection is a good candidate for the Active Managerial Control award. EHS observed the following good active managerial control systems in place: expiration time labels placed on santizer solution buckets, good date marking, food labels placed on each item in the Walkin Cooler with date and time of preparation and discard date and time (which allows easy verification for rapid cooling), good cooling methods to facilitate rapid cooling (use of speed rack and uncovered flat metal trays), cold and hold holding temperature logs maintained and easily accessible documentation on the Yellowfin Tuna used in sushi. Currently only yellowfin tuna and cooked crab meat and used inside sushi rolls. Neither item requires records of parasite destruction: as yellowfin tuna is exempt and the crab meat is cooked.
No violation noted during this evaluation.

  • Fish Served Raw RTE / Records Creation & Retention of Freezing For Parasite Destruction
    Observation: No records of freezing for parasite destruction are maintained for the following raw, raw marinated, partially cooked or marinated-partially cooked fish that are served or sold in ready-to-eat form: Tuna sushi, Crab sushi
    Correction: If raw or partially cooked fish are served or sold in ready-to-eat form, the person in charge shall record the freezing temperature and time to which the fish are subjected and shall retain the records for the frozen fish at the food establishment for 90 calendar days beyond the time of service or sale to a consumer. If the fish are frozen by a supplier, a letter the fish supplier stipulating that the fish is frozen as required shall be provided.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:pulled cooked chicken=51-F
    Correction: chicken wanton =48-F, ground chicken wrap mix =48-F
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The compartments under the deep fryers were noted to be in need of cleaning. Grease and debris were noted on the surfaces inside the compartments.
    Correction: Clean the surfaces of the compartments more often to prevent debris and grease accumulation.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the floor under the cooking equipment in the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

  • Utensils That are In-Use / Between-Use Storage
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Measuring scoops were found stored on bulk food with the handles laying directly on the food.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Veggie with minced chicken = 46-F, plain minced chicken = 45-F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. (relocated)
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There were no temperature measuring devices located in some of the refrigerators on the cookline.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Operation
    Observation: One of the drawers on the cookline cold prep tabled is not in good repair. This drawer is loose and almost detached and was observed tied with a plastic to hold it in place.
    Correction: Repair this unir properly so that it is able to maintain foods at or below 41°F.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of one of the cookline cold prep table refrigerator are broken.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Water / Pressure / Quantity and Availability
    Observation: Hot / Cold water under insufficient or inadequate pressure was found to serve all of the handwashing sinks in the restaurant.
    Correction: Water under pressure shall be provided to all fixtures, equipment, and nonfood equipment that are required to use water.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F (repeated violation)
    Observation: Water from the handwashing sinks in the cooking, food prep and restrooms areas was measured at a temperature less than 100°F. Observed temperatures were 62-F,79-F, 88-F, and 94-F
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet. Adjust or repair the sinks to provide adequate hot water temperature for proper hand washing.
  • Critical: Common Name on Working Containers of Toxics (Cleaners, Sanitizers) (corrected on site)
    Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name. Unmarked spray bottles with cleaning chemical solutions were observed without labels to identify contents.
    Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
