Today a ROUTINE INSPECTION will be conducted in conjunction with a complaint inspection following a complaint received that patron had had eaten at this facility several weeks ago and had Campylobacter. Thank you for accompanying me during this visit as your participation allows me to assess areas at your facility where further clarification may be needed .
NOTE: At this inspection it was observed that all staff were washing hands and using gloves as required. As per manager no staff had called in sick or displayed symptoms of GI illness in the last 2 months. Employee Health Policy documents are on file and this is reviewed with staff on a regular basis. At this inspection no definitive conclusion could be made to associate this facility as being causative for the complainant's illness.

A Follow up inspection was conducted today to assess compliance with extermination and control of flies at the facility and presence of a certified food manager with valid card at all times when facility is operational. These items were listed at the August 5 inspection. All noted items have been corrected , no flies were observed and the facility has two certified food managers at the time of this inspection.

    Correction: The person in charge failed to state the required temperatures and holding times for the potentially hazardous food items when (1) in refrigerated storage, (2) in hot holding, (3) being cooled and/or (4) being reheated. PERSON IN CHARGE AT THE BEGINNING OF THE INSPECTION WHEN QUESTIONED BY THE INSPECTOR HAD NO KNOWLEDGE OF COLD HOLDING, HOT HOLDING, COOKING OR REFRIGERATOR AMBIENT TEMPERATURES.

    Correction: The person in charge failed to explain the correct procedures for cleaning and sanitizing utensils and food-contact surfaces of equipment. PERSON IN CHARGEAT THE BEGINNING OF DID NOT HAVE KNOWLEDGE OF TYPES OF SANITIZERS, SANITIZER THE FACILITY IS CURRENTLY USING, PROCEEDURES OF SANITATION OR THE TYPES OF TEST STRIPS AND THEIR READINGS.

    Observation: The following equipment/utensils were observed soiled to sight and touch: OBSERVED KNIVES STORED ON THE PREP TABLES AND ON SHELF ABOVE THE PIZZA REFRIGERATOR WITH FOOD DEBRIS, GRIME AND OTHER SEDIMENTS ON THEM.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. ASKED THE CFM TO WASH, RINSE AND SANITIZE THE KNIVES.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection. OBSERVED THAT THE PERSON IN CHARGE AT THE BEGINNING OF THE INSPECTION HAD AN EXPIRED CFM CARD (6/17/2015)
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. A CFM WITH VALID FOOD MANAGER CARD ARRIVED APPROXIAMTELY 30 MINUTES AFTER THE INSPECTION.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found (repeated violation)
    Observation: Methods are not being used to control pests. OBSERVED A LARGE NUMBER OF FLIES ON THE PREMISES, IN THE FRONT AREA, ON THE PREP TABLES AT THE PIZZA PREP REFRIGERATOR AND NEAR THE 3 VAT SINK AREA.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. DIASCUSSED WITH MANAGER, HOUSE FLIES CARRY MANY DISEASE AND ARE A HEALTH HAZARD. PROFESSIONAL PEST CONTROL COMPANY IS TO BE CALLED WITHIN 5 DAYS AND THE FLIES EXTERMINATED, ENSURE THAT DOORS AND WINDOWS ARE CLOSED SECURELY TO PREVENT ENTRY OF FLIES INTO THE PREMISES. FOLLOW UP WILL BE CONDUCTED BY THE HEALTH DEPARTMENT WHERE RECORDS OF EXTERMIANTION WILL BE REVIEWED AND COMPLIANCE IN EXTERMIANTION OF FLIES WILL BE ASSESSED.
