    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Warewashing Equipment, Determining Chemical Sanitizer Concentration (corrected on site)
    Observation: The chemical sanitizer was too strong. Sanitizing solution observed >200ppm, CFM corrected to 100ppm Cl.
    Correction: Obtain a chlorine chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the hood filters had accumulations of dust and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

The pastor went to City Hall to change the name of the church and they informed him that the address for the facility is incorrect. Correct address and new name for facility added into Healthspace and new permit with correct address will be reprinted.

    Observation: Hood filters observed dusty, noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

If unable to repair the microwave, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The nonfood contact surface of the can opener blade and top shelf of grill had accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Floor or floor covering around grease trap is not smooth and easily cleanable.
    Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.

    Observation: Shelf liner used in cabinet and small areas of exposed wood.
    Correction: Discontinue use of shelf liner for this purpose. Paint or seal all exposed wood. All non-food contact surfaces must be smooth, durable, and nonabsorbent, to prevent deterioration and to maintain an easily cleanable surface.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket of the Artic Air refrigerator is damaged.
    Correction: Repair or replace the Artic Air door gasket in accordance with the manufacturer's specifications.
  • Equipment and Utensils - Good Repair and Calibration
    Observation: Hood filters were observed in a state of repair and condition preventing the equipment to be used as designed.
    Correction: Close gap in hood filters to provide proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter.
  • Light Bulbs Protective Shielding
    Observation: Light bulbs in kitchen not fully shielded, coated, or otherwise shatter-resistent.
    Correction: Adjust bulb shields to properly cover entire light. Shield or replace light bulb with a coated or shatter-resistent bulb.
