Today I conducted a routine inspection and discussed with management the following: 1. Management should reiterate the Employee Health Policy and Personal hygiene with all staff frequently. 2. Management should properly reflect the Consumer Advisory on the web site menu for the banquet kitchen. Items will change seasonally. 3. Management should calibrate all digital food thermometers at least once a week in an ice water bath to 32f to ensure accuracy. 4. Management should provide a location for storage of coats, purses, etc for temporary staff if lockers are not provided. 5. Management should implement Active Managerial Control at all times. At this time the heat sanitizing dish machine located on the first floor is not in use. It is only used in the event of a large banquet. Management shall ensure it is functioning properly to wash and heat sanitize according to manufacturer's specifications before being used. Condiments Protected from Contamination Observation: The condiments in the employee dining area are not stored or dispensed in a manner to prevent contamination.
Correction: Condiments shall be protected from contamination by: 1) being kept in dispensers that are designed to provide protection, 2) food displays provided with proper utensils, 3.)original containers designed for dispensing, or 4) individual packages or portions. DISCUSSED WITH MANAGEMENT. MANAGEMENT WILL PROVIDE PROPER LIDS FOR THE CONDIMENT CONTAINERS.
Fish Served Raw RTE / Records Creation & Retention of Freezing For Parasite Destruction Observation: No records of freezing for parasite destruction are maintained for the following raw, raw marinated, partially cooked or marinated-partially cooked fish that are served or sold in ready-to-eat form: 1. Any seafood items on the menu. Items may change seasonally.
Correction: If the fish are frozen by a supplier a letter from the fish supplier stipulating that the fish is frozen as required according to 3-402.11 shall be provided. DISCUSSED WITH MANAGMENT AND THEY WILL OBTAIN THE PARASITE DESTRUCTION INFORMATION FROM THE SEAFOOD SUPPLIER AND EMAIL ME WITHIN 10 BUSINESS DAYS.
Farm Raised Fish Served Raw RTE / Written Statement from Supplier Observation: The following farm-raised fish served raw or partially cooked were observed without a written statement from the supplier stating that the fish were raised and fed according to 3-402.11(B)(3): 1. Salmon products.
Correction: Contact your fish supplier and ask for a written statement stating that the fish were raised in net pens if in open water OR land-based operations AND fed formulated feed that contains no live parasites. EMAIL a copy of this letter to your inspector and keep this statement on site at the food establishment for 90 days beyond the time of service of the fish. DISCUSSED WITH MANAGEMENT. MANAGEMENT WILL CONTACT THE SEAFOOD SUPPLIER AND OBTAIN THE REQUIRED INFORMATION AND EMAIL ME WITHIN 10 BUSINESS DAYS.
Mops in Air-dry Position Observation: Observed that mops are improperly stored between use.
Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies. DISCUSSED WITH MANAGEMENT. MANAGEMENT WILL TRAIN STAFF TO PROPERLY STORE MOPS WHEN NOT IN USE.

This visit was conducted to do a Risk Factor inspection. No risk factor violations found during this inspection. Facility was well organized and good date marking observed. Active Managerial Control and Health Policy discussed. No violation noted during this evaluation.

Risk Factor
This visit was conducted to do a Follow up for a Pre opening done on 3//25/15. All corrections have been made. Approval is hereby granted for the issuance of a Health Department permit. This inspection report shall serve as your permit until the official permit is obtained. A routine inspection will be conducted in approximately 30 days. No equipment additions/changes/replacement are allowed without Health Department approval. No violation noted during this evaluation.
