  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: 4 drawer preptop cooler, 1dr cooler #1, 1dr cooler #2, walkin cooler
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the sanitizer buckets with a concentration of 0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. Food employee reset solution to 300ppm
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM arrived during inspection

    Observation: Openings to the exterior of the building are present along the delivery double doors
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.

    Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit: 1dr cooler, delfield 4drawer preptop cooler
    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.

The purpose of today's visit was to conduct a routine inspection.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The self closer on the walkin cooler is not in good repair.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
07/17/2014Routine
The purpose of today's visit was to investigate a complaint. Health Department received a complaint on March 19, 2014. Complainant alleges that on March 18, 2014 around 1 pm that a female employee was handling trash and serving food at the same time without handing their hands. Complainant also alleges that employee refused to identify their self. EHS discussed complaint with CFM/manager on duty today. CFM was not aware of the complaint. EHS observed facility has two fully stocked hand sinks in kitchen. One hand sink is along the serve line and one hand sink next to the three compartment sink. Both hand sinks have hand washing signs stating when hands must be washed. There are also hand sinks in each public restroom, both properly supplied. EHS discussed facility's hand washing policy with manager and EHS recommended to manager to review the hand washing policy with all employees. EHS also answered questions regarding the Food Code from the manager unrelated to today's complaint. Complaint cannot be confirmed at this time.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: OBSERVED ICE SCOOP OF ICE MACHINE IN THE BACK AREA PLACED DOWN ON TOP OF THE ICE MACHINE .
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. CORRECTED: CFM REMOVED THE ITEM FROM THE AREA.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: OBSERVED SKIM MILK AT 45F AND 2% MILK AT 43 IN THE DAIRY DISPENSING HOPPER.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.CORRECTED: CFM REDUCED THE TEMPERATURE SETTING OF THE HOPPER.
  • Critical: Equipment / Food-Contact / Design / Ease Clean
    Observation: The food-contact surface of the following equipment contains cracks, chips, or pits and can not be easily cleaned: OBSERVED THAT THE SURFACE OF DOUGH MAKER WAS CHIPPED AND RUSTED IN MANY AREAS.
    Correction: Multiuse food-contact surfaces shall be: 1) smooth, 2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections, 3) free of sharp integral angles, corners, and crevices, 4) finished to have smooth welds and joints, and 5) accessible for cleaning and inspection. Cleaning and inspection shall be completed by one of the following methods: (a) without being disassembled, (b). by disassembling without the use of tools, or (c) by easy disassembling with the use of handheld tools.
