Above the three compartment sink, there was one lid with a crack on the food contact surface. On the green rack in the food prep area, there were four containers with cracks and pieces missing from them. COS-all items were voluntarily discarded.
- KDA_4_60111A - --Stored as clean in a container on the green rack in the food prep area, were four brown serving spoons with sticky debris on them. On the same green shelf, stored as clean were three containers that had an accumulation of black grease and debris on the ledge of the container. Person in charge said, this is what they use on steam table to hold the hamburger patties. COS- items were placed for rewashing.

- KDA_4_60212B - --The warming oven, has dried on food debris. Person in charge said she knows it's to be cleaned daily. She said it only gets done once a week. She will see that it gets cleaned daily.
