The foods were cooked one hour prior then placed in the hot holding unit, as per employee. (COS-discarded).
- KDA_4_204112B - --There was not an internal thermometer located in the front counter hot holding unit. (COS-thermometer was placed in the hot holding unit).
- KDA_4_60111A - --There was accumulation of grease deposits on the food contact surface of three wire racks stored as clean on the south clean dish shelf in the kitchen. (COS-placed in the 3-vat sink to clean).
- KDA_6_501112 - --There were four dead mice in the bait station on the west wall in the northeast storage room. The facility did not have a recent invoice from the pest control company.
- KDA_7_10211 - --A blue solution was stored in a spray bottle labeled as degreaser in the southeast corner of the kitchen. The solution in the bottle was glass cleaner, as per employee. (COS-labeled).
- KDA_7_20111A - --Three spray bottles of all purpose cleaner were stored with the nozzles facing an open case of honey mustard sauce under the counter west of the pizza hot holding unit. (COS-chemicals were moved).
- KDA_7_20111B - --An aerosol can of stainless steel cleaner was stored on a shelf over Champs sauces (sweet and fiery, jalapeno mustard) and bagged cheese sauce west of the grill. (COS-chemicals were moved).
- KDA_7_20711B - --A bottle of Kaopectate was stored over ready to eat burritos in the kitchen walk in cooler. (COS-Kaopectate was removed from the cooler).
- KDA_Defacto - --3-vat sink: 200 parts per million quaternary ammonia --Hot holding cabinet on the front counter: fried chicken 134 F, potato wedges 135 F, corn dogs 136 F --Kitchen walk in cooler: ready to eat white gravy 68 F.
