KDA_3_30511A1 - --There are 2 boxes of individual packets of ketchup and mustard being stored directly on the floor below the 3 compartment sink behind the bar.
KDA_3_50116A2 - --In the WIC there was a tub of roasted garlic (keep refrigerated) at 46.1F. There was also a package of sliced cheese at 46.5F. The owner said it has been in the WIC less than 4 hours. The ambient air temperature of the unit was reading 41.0F. COS-moved potentially hazardous food to another reach in cooler.
KDA_3_50118A2 - --In the True RIC there were plastic food grade containers of cooked ground beef, re-fried beans, nacho cheese sauce and mexican rice without dates. There was also a plastic food grade container of cooked grilled chicken and a ziplock back of a cooked chicken strip without dates. The owner said they were made or opened on 10/2/15. COS-all items were date marked.
KDA_4_60111A - --In the True RIC there was a purple plastic food grade container that had stickers left on the outside and it was being used to store grilled chicken. On a shelf below a metal prep table there was a purple plastic food grade container with stickers left on the outside and stored clean. There was also one dark blue plastic lid with oil residue on it and stored clean. In the plastic utensil holder there was one metal fork with dried food debris on the food contact surface side. COS-moved all items to the ware wash area.
KDA_4_90311A - --In the storage room there were 5 boxes of single use disposable gloves being stored directly on the floor. COS-placed on a shelf.
KDA_5_20511B - --The handsink behind the bar was used as a dump sink. There were 3 straws in the sink drain. COS-removed straws.
KDA_5_20515B - --The utility sink in the storage room leaks at the faucet and below the sink when water is supplied. The owner said she does not use the utility sink. Disposes the mop water down the floor drain. At the 3 compartment sink behind the bar, the hot water handle leaks when water is supplied.
KDA_8_30411A - --The current 2015-2016 license is not posted. The owner said she never received it. KDA 61 contacted the office and a new license will be issued and mailed.
