KDA_3_50116A1 - --In the bottom open warmer, there was a bean burrito holding at 125.1F. The employee said they had been there since 10:45am. (cos-most items in case are being marked with times for time as a control. The employee started using time as a control on the items, since they are cooked at the other store and brought over)
KDA_3_50116A2 - --In the prep area, there was a pan of cut lettuce stored on ice holding at 50.9F. The employee said it was placed on ice at 10am. The ice only covered the bottom surface of the pan holding the cut lettuce. (cos-cut lettuce cooled down)
KDA_4_50111B - --The gasket on the bottom side of the walk in door (2nd from right) is torn.
KDA_4_60111C - --There is minor dust buildup on the ceiling near the fan blowers in the walk in cooler.
KDA_4_60211E4 - --There is minor mold build-up inside of the pop nozzles at the soda fountain machine. There is minor mold build-up on the inside ledge of the ice machine.

4-302.14 - --The establishment did not have chlorine test strips available at the time of the inspection. COS-left chlorine test strips.
KDA_3_30212 - --To the left of the make table there was a working spray bottle with clear liquid and not labeled. An employee said it was water. COS-labeled bottle.
KDA_3_30412C - --The large pizza cutter was being stored wedged between the make table and the counter which is not a clean/sanitized surface. COS-moved it to the ware wash area.
KDA_3_30511A1 - --In the back storage room there were 2 boxes of potato chips being stored directly on the floor.
KDA_3_50113A - --There was an aluminum container of Breakfast Bake (sausage & gravy) thawing on the counter below the pizza oven. An employee said it was placed out around 8:00am (less than 4 hours). COS-placed in the WIC.
KDA_3_50116A2 - --On top of the True RIC there was a plastic food grade container of sliced tomatoes at 58.9F and a plastic food grade container of cut lettuce leaves at 67.8F. They were not on an ice bath and an employee said she placed them out at 9:30am and tempted around 10:55am (less than 4 hours). COS-placed items on an ice bath.
KDA_3_50118A2 - --In the WIC there were 2 ziplock bags of shredded lettuce without a date. An employee said they were placed in there on 10/27/15. COS-voluntarily discarded the product.
KDA_4_50111B - --The middle door gasket to the WIC is torn. This does not provide a good seal.
KDA_4_60111A - --On the shelf across from the 3 compartment sink there was one red square plastic food grade lid with a sticker label left on it and stored clean (can hold bacteria). COS-moved to the ware wash area.

KDA_4_60211E4 - --There is dried food debris build up on the Cappuccino machine dispensers. The person in charge said they get clean every other day. There is minor mold build up inside of the pop nozzles at the soda fountain machines.
