- 2-301.14(E) - --Employee handled pizza dough with bare hands after touching soiled handles of the walk in cooler (WIC) and reach in cooler (RIC)without washing his hands. Corrected on site (COS)- discarded pizza dough. Employee went from handling dirty dishes to handling clean dishes without washing her hands. COS- employee rewashed the dishes and washed her hands before putting the clean dishes away.
- KDA_2_30114D - --Employee ate a bite of food and then cut pizza immediately afterwards without washing his hands.
- KDA_2_40111 - --Employee was eating at the pizza cut table.
- KDA_3_10111 - --Located in the McCall RIC, there were 10 pans of pizza dough that were separated with lids that were not cleaned properly. The pizza dough had food debris from the unclean pans in it. COS- discarded dough.
- KDA_3_30111B - --Employee was cutting lettuce with a knife and touching the lettuce with his bare hands. COS- discarded lettuce.
- KDA_3_50116A1 - --Located at the pizza buffet, there was a pepperoni pizza at 114F and a sausage pizza at 115F. Both pizzas had been on the buffet for less than 20 minutes. COS- took pizzas off of the buffet line.
- KDA_4_20211A2 - --Located in the dish room, there were 15 lids that were chipped or cracked.
- KDA_4_60111A - --Located on a rack above the MT, there were pizza crust covers stored as clean that were soiled with dried food debris. Located in the McCall RIC, there were pizza pan dividers that were soiled with food debris that were used to separate pizza pans that had fresh pizza crusts in them. The food debris on the pan dividers was contacting and contaminating the dough. COS- discarded 10 pans of pizza dough, moved soiled pan dividers to wash area. Located in the dish storage room, there was a dish rack full of pan dividers, pizza pans, and lids stored as clean. Approximately 75% of all dishes on the dish rack were soiled with an accumulation of food debris and grease. There was grease dripping from the pizza pans onto the floor. Located in the dish room, there was a dough prep table with two containers filled with oil. The containers were soiled with an accumulation of oil and food debris. Located in the dish room, there was a container used to store clean lids. Most of the lids in the container were soiled with sticker residue.
- KDA_4_60111C - --The McCall RIC had an accumulation of food debris on the bottom shelf. The shelves and tables located in the dish storage room were soiled with an accumulation of food debris and grease. The fan guards and shelves of the WIC were soiled with an accumulation of dust and dried food debris.
- KDA_4_60211E4 - --The soda fountain located in the waitress station was soiled with an accumulation of mold under the soda nozzles and in the ice chute.
- KDA_4_90311B - --Located in the dish room, there was a rack of dishes stored as clean. Most of the dishes on the rack were stacked wet.
- KDA_4_90411A - --Employee was putting clean spatulas in a storage container using her bare hands and touching the food contact surface of the spatulas. COS- rewashed spatulas.
- KDA_5_20515B - --The kitchen hand washing sink faucet sprays out when turned on. The drain in the kitchen hand sink is draining slowly. The hand washing sink faucet located in the waitress station is leaking.
- KDA_5_40311 - --There was a large amount of grey sewage matter located on the walls, under the ware wash machine, and under the counter at the waitress station. Person in charge (PIC) said that initially the grease trap was clogged and backed up into the sewer line, further clogging the line. Employee indicated that the drain had been repaired this morning.
- KDA_6_50112A - --The walls and floors of the kitchen area and dish storage room were soiled with an accumulation of food debris, grease, and sewage backup. The vent hood above the pizza ovens was soiled with an accumulation of grease and dust.
- KDA_7_20111B - --There was a container of rinse aid and a container of detergent being stored on top of the ware wash machine over the side of the machine where clean dishes come out.
- KDA_8_40411 - --There was sewage backup located in the kitchen area and under the wait station counter. COS- voluntary closure.
- KDA_Defacto - --chlorine sanitizer concentration in ware wash machine: 50 ppm test strips available --Located at the back of the kitchen, there was large container full of water and chemical used for soaking plastic equipment. When the lid was opened, a foul odor came out. Employee did not know when the last time the water was changed.
