KDA_3_30212 - --The sugar bin, flour bin, and salt container did not have labels. COS. Educated staff and items were labeled. KDA_3_30412A - --Located in the sugar bin, the scoop was laying down and the handle was touching the sugar. COS PIC moved scoop. KDA_3_50118A2 - --Located in the WIC, there was one open gallon of milk and one open quart of cream with no open dates. PIC said both items were opened on August 5, 2015. COS. Items were dated. KDA_4_20211A2 - --Located in the drawers for clean utensils, there were 3 utensils with cracks and melted/mangles handles and were not cleanable. COS. Items were placed in trash can. Located on the shelving for eating utensils, the plastic containers are broken, cracked, and not cleanable. PIC is working on replacing the items. KDA_4_50111B - --The WIF door and the WIC doors have cracks and are falling apart wit loose panels. The cooking stoves have broken doors and the ovens do not work. KDA_4_60111A - --Located on the shelving for dry clean dishes, there were 3 of 50 pans with sticky tap residue. COS. Items were taken to dish area to be cleaned. The two table mounted can opener blades had dried food debris. PIC did not know when they were last washed. COS. Items were taken to dish area to be cleaned. The meat slicer had dried food debris on the blades and pushing block. COS. PIC had an employee start cleaning the machine during inspection. KDA_4_60111C - --The cooking stoves on the banquet side have an accumulation of grease and food debris. The 60 quart floor mixer has a large accumulation of dried food debris. The 30 quart mixer has an accumulation of dried food debris. The flour and sugar bins have an accumulation of dried food debris on the exterior. The WIC and WIF doors have an accumulation of black and dried food debris. The shelving racks throughout the kitchen, approximately 15, have a large accumulation of rust, grease, and dried food debris and are heavily corroded. KDA_4_60212B - --The microwave located in the waitress statin, has dried food debris on the inside walls and ceiling and bottom. KDA_4_70211 - --The dish machine is a hot water sanitize machine. The employee was using machine and had ran 5 racks through machine. The water tested at 125F using lollipop thermometer. PIC said dish machine is being repaired today. COS. Educated staff on sanitizing dishes. Explained to go ahead and wash dishes with machine, but all dishes will have to be dipped in a sanitize solution, including the clean dishes just washed. A solution of QUAT sanitizer was prepared and used. KDA_4_90311A - --Located in the dry storage room, there were 2 cases of single use drinking cups sitting directly on the floor. COS. PIC placed items on shelving. KDA_6_30311A - --There is one light not working above the cooking area. KDA_6_30511B - --Located on the shelving for creamers and sugars, there were 3 personal drinking cups and 1 package of medicine stored above the cases of single use creamer. Cos. Educated staff on designation an area for personal items. Items were moved. KDA_6_50111 - --Located in the cooking area of the kitchen, there are broken and loose wall tiles falling off the walls. KDA_6_50112A - --The exterior doors of the employees toilet rooms have an accumulation of greasy, sticky, black residue. The floors in the storage rooms, dishwasher area, and general area have a large accumulation of greasy, stick, black residue. KDA_6_50116 - --The wet mop was laying scrunched in the bottom of the mop basin. COS. Mop was hung on wall t allow it to air dry. KDA_7_20111B - --Located on the shelving in the wait staff area, there was one bottle of anti-bacterial all purpose cleaner sitting on the shelf directly above 1 case of single use creamers. COS. Chemicals were moved. KDA_7_20212A1 - --There were 3 cans of non-approved, non food sage "Flying insect killer" located in the kitchen areas. PIC said they were having issues with gnats and flies and the were spraying the drains and floors in the kitchen areas. Item EPA #46515-48-8845. COS. Educated staff and PIC placed cans in the office to only be used in outside areas.
