KDA_4_90311A - --Located in the kitchen next to the storage shelves, there were boxes of deli containers, clam shell containers, and pizza boxes that were being stored on the floor. KDA_4_90311B - --Located on the storage shelf in the kitchen, there were two plastic food containers stored as clean that were stacked wet. KDA_5_20313A - --There is no mop sink available to dispose of mop water. COS- field approval. Disposing of mop water down beveled floor drain.
