- 4-302.14 - --The establishment does not have any quaternary ammonia test strips. I discussed proper concentration levels and preparation with the kitchen manager.
- KDA_3_30211A1b - --In the WIC a carton of raw shell eggs was stored on a shelf over bottles of RTE water. Also, packages of hot dogs were stored in direct contact with packages of raw bacon and cartons of raw shell eggs. COS, items moved to an approved location.
- KDA_3_30212 - --On a shelf over the doughnut station there was a bottle of clear liquid that was not labeled. The PIC said it was water. COS, labeled.
- KDA_3_30414B1 - --The quaternary ammonia sanitizer solution used for wiping down tables tested at more than 500ppm. COS, remade to the correct concentration.
- KDA_3_50118A3 - --In the RIC below the sandwich MT #1 there were 2 containers of taco meat that were date marked open on 12/03 and date marked to discard on 12/10. COS, employee dated correctly and I educated employee about proper date marking.
- KDA_3_50119B2 - --In the hot box # 1 cooked potato wedges had a temperature of 114 F. The employee said the items put in the hot box are held for 1 hour and then discarded. At 11:15 she said the potato wedges were put in the hot box at 10:30 but she forgot to mark the time. COS, she chose to discard.
- KDA_3_60211C - --The bulk pastry case does not have a label with the common name of items or their ingredients. The allergens are listed on the case.
- KDA_4_10111A - --In the prep room mesh bags of onions are stored with onions in direct contact with a non-food grade Sterilite container. COS, removed from container and employee educated.
- KDA_4_20211A2 - --A food storage container on an in-use container was chipped on the edge. Also, 2 spoons stored as clean in the utensil drawer were pitted on the food contact surface. COS, all items discarded.
- KDA_4_50213B - --The creamer dispensing tube on the half and half is longer than 1 inch and not cut diagonally.
- KDA_4_60111C - --In the kitchen WIC the fan covers are dusty. There is food debris around the welds on the shelf holding the pizza pans.
- KDA_4_60211E4 - --The ice chute at the soda fountain had mold and mineral deposits inside of it.
- KDA_4_90311B - --Food storage containers stored as clean over the 3-vat sink were stacked wet. Educated employees about proper drying methods.
- KDA_5_20511B - --At the handwashing sink in the kitchen on the east side there were 2 sheet pans and a food storage container covering the sink basin that was preventing employees from washing their hands. COS, moved.
- KDA_5_20515B - --The AVB is leaking on the mop sink. Also, the pipe below the handwashing sink at the beverage station has mold around a connector and is leaking into a bucket that had a mold film across the top of the water.
- KDA_5_501113 - --The lids on the dumpster are open allowing pests to enter.
- KDA_6_30114 - --At the 2 handwashing sinks in the kitchen a sign is covering the sign to notify employees to wash their hands. There is no sign to notify employees to wash their hands in either of the restrooms. Employee left handwashing stickers.

At 11:00 a kitchen employee said the gravy was removed from hot holding to cool at 10:00 AM. Note: I discussed proper cooling procedures with employee. --Proper hot holding temperatures: Hot box 1: chicken 142 F Hot box 2: pizza 148 F
- KDA_KSA65_657a - --In the drug retail isle the following medications were for retail sale with the corresponding expiration dates: 2 boxes of Alka Seltzer 7/15 1 box of Emergen-C 10/14 3 boxes of Pain Reliever 10/15 2 boxes of Bayer 11/15 COS, removed from retail sale.
