  • KDA_Defacto - --The three compartment sink was not set up. --Walk in cooler ambient 38.9F. Make table, mozzarella cheese 40.9F. Reach in make table, right side cheese sauce 40.6F, left side ranch dressing 40.9F.

  • 4-302.14 - --The quat test strips were old and in a tangle and not useable. Person in charge said they will order new.
  • KDA_3_30212 - --Above the prep table on the south wall, there were two containers that were with out a common name. Person in charge said they were sugar and cinnamon and sugar. COS-containers were labeled.
  • KDA_3_30414B1 - --The wet sanitizing cloth was stored above the sanitizing bucket on a shelf. COS-cloth was stored in sanitizing bucket. The sanitizing cloth bucket had 400 plus PPM Quat. COS-sanitizer was made to label requirements of 300PPM.
  • KDA_3_30511A1 - --In the walk in freezer, there was one box of fully cooked sausage crumbles and three boxes of crispy marshmallow treats on the floor. COS-boxes were to a shelf.
  • KDA_3_50116A2 - --In the make table, cooked deli ham slices were at 48.6F. Employee said the ham was taken out of the freezer at 5:00AM and put in a bowl of hot water to thaw.Person in charge wanted to discarded the ham. COS-ham was discarded.
  • KDA_3_50118A1 - --In the reach in make table, an open container of Canadian bacon had open date of 9/30 and discard date of 10/7. Canadian bacon was held over seven days. Educated on date marking. COS-Canadian bacon was discarded.
  • KDA_4_20211A2 - --Being washed, rinsed and sanitized were four containers that were pitted and scarred. In the utensil drawer were two rubber spatulas that were missing pieces. COS-rubber spatulas were discarded. The containers will not be used to store food directly in contact with container, they will put a food safe bag in it.
  • KDA_4_50111A - --The door to the reach in freezer, where the ice cream is stored, is not automatically closing. The ice cream products have an accumulation of ice build up.
  • KDA_4_60111A - --Underneath the prep table on the south wall, in a drawer stored as clean serving utensils had grease and food debris on them. COS-Utensils were placed at the three compartment sink.
  • KDA_4_60111C - --In the walk in freezer, there is food, food debris, trash and dirt under the shelves. the fans in the walk in freezer and cooler had some dust. The stack drawer holding the serving utensils had an accumulation of grease and dirt on the handle.
  • KDA_4_60211E4 - --The ice machine ledge has a small amount of red and black mold.
  • KDA_4_70211 - --While employee was washing dishes the sanitizing solution in the three compartment sink was at 100PPM quaternary ammonia. COS-The solution was corrected to 300PPM as the direction on the label.
  • KDA_4_90311B - --On a shelf above the three compartment sink, there were two containers stacked wet. COS-containers were moved to three compartment sink.
  • KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
  • KDA_5_20515B - --The right faucet at the three compartment sink is leaking where the handles meet.
  • KDA_5_50117 - --The unisex restroom, does not have a trash receptacle with a lid.
  • KDA_6_50111 - --Floor tile is missing through out the establishment.
  • KDA_6_50112A - --The vents above the pizza warmer the vents have an accumulation of dust. On the reach in freezer, there is mold on the bottle metal ledge on the outside of the unit.
  • KDA_6_50119 - --The unisex restroom door was open during business hours. COS-door was closed.
  • KDA_Defacto - --Frozen food is frozen. --In the reach in make table, fully cooked diced chicken 42.8F, make table, pizza sauce 40.5F. Walk in cooler Fully cooked potato wedges 38.8F. On the coffee counter, French vanilla creamer 33.7F. --In the warmer, breakfast pizza 174.4F. --Kitchen employee did not know she had food temperature thermometer. Inspector saw it in a container.
