KDA_4_204112B - --No thermometer in the white reach in cooler in the kitchen. KDA_5_50117 - --No covered trash can in the women's restroom. KDA_6_20214 - --The self closing device on the women's restroom door is inoperable. KDA_6_20215A1 - --Upstairs (which is not going to be used and is separated) there is a gap on the wall of about two inches on the east side, large enough for pest entry. This can allow pests to get into the building. KDA_6_20215A3 - --The west kitchen door has a gap of about 1/2 inch on the top left corner facing the door while in the kitchen, large enough to allow pest entry. KDA_6_30111 - --No soap at the bar handsink (soap placed at sink) KDA_6_30112 - --No paper towels at the bar handsink (towels placed at sink). KDA_6_30113 - --Handsoap was at the produce prep sink (moved to the bar handsink). KDA_6_30114 - --No handwashing signs at the four handsinks. KDA_6_501114A - --Upstairs (which is not going to be used and is separated) is a lot of junk and numerous items. These should be removed as it can be a habitat for pests. In a storage room west of the kitchen, is a large broken down freezer. KDA_6_50112A - --On a shelf at the cashier's stand, a few old rodent droppings. Upstairs (which is not going to be used and is separated) dirt, debris, and a few old rodent droppings are present. Also, by the gap mentioned above is what appears to be bird nesting materials. Clean the upstairs and monitor periodically for pest activity. KDA_7_10211 - --On a shelf, two unlabeled spray bottles, one containing a blue liquid and the other a green liquid (Windex and Mr. Clean as per owner)(labeled).
