4-302.14 - --There are no test strips in establishment. Educated on needing them because they wash, rinse and sanitize pop nozzle, coffee pots and tea pot. Person in charge said she will order them today. KDA_2_30114H - --Employee had gloves on, went to reach in cooler, opened it and got out a container of chopped ham. Employee then put on new gloves and touched ready to eat chopped ham, peppers and onions. Educated on hand washing. COS-food was discarded. Employee washed hands and put on new gloves. KDA_3_30412B - --Under the make table in the kitchen, there is large white container with chicken batter powder in it. The scoop handle was touching the food surface. COS-scoop was removed. KDA_3_30414B1 - --In the cloth bucket, the bleach sanitizer was 10PPM. COS-new water was made and bleach was 100PPM KDA_3_50116A2 - --On the make table sitting on ice: diced tomatoes 58.3F, cooked sausage 57.7F, shredded cheese 54.3F, mushrooms 64.8F. Person in charge said these items had been prepped and out on make table since 6:30. Educated on the need for ice to be a full as containers. COS-items were placed in reach in cooler. KDA_3_50118A1 - --In the walk in cooler, there was a pan of chili that was dated 9/16/15. It was held more than 7 days. COS-Chili was discarded. This was the only date marking violation, so issued a FWL. KDA_4_20211A2 - --In the waitress area on the make table, there were five salad bar containers that had cracks along the food contact surface. There are ten steam table plastic lids that are cracked. Employee said they do not use these. They have to keep them for inventory with owner. COS-bowls were removed and will not be used for food. KDA_4_50111B - --On the ice machine, the seal is loose. The handle on the reach in cooler is kept closed with a screw driver. KDA_4_60111A - --There was one container in the waitress area with food debris in along the food contact area. COS- bowl was put at ware washing machine. KDA_4_90311A - --Sitting on top of the ice machine, there were two ice buckets, sitting with food contact surface face up. COS-buckets were washed and placed upside down in two containers. KDA_6_20116A - --The walls beside the fryers, have paint peeling and are not easy cleanable. KDA_6_50111 - --Near the ice machine, the ceiling is cracked. The floor is missing the coating in areas through out the kitchen KDA_6_50112A - --There is an accumulation of dust in the following areas: behind the ice machine, ceilings through out the kitchen and back prep area, walls in the kitchen, vents in the doors to the kitchen, on the light covers. The back of the grill has an accumulation of dust. KDA_Defacto - --Frozen food is frozen. --On the grill: egg cooking 170.9F, in the oven: Salisbury steak 197.3F. On the stove: soup 173.3F. --On the grill: hashbrowns 193.3F, gravy in crock pot 138.7F, In steamer: white gravy 134.9F. --On the salad bar: peas 37.6F.
