  • Violation: 15-32-1 Observed pizza prep reach-in cooler gasket torn/in disrepair. On call back gaskets were torn.

On call back inspection spoke to cashier/waitress she called operator she stated to me the food manager card was in office and door was locked. I spoke with operator on phone and he also stated to me food manager card was locked in office.
