One slotted spoon stored as clean on hook above food prep sink with visible food debris on surface. COS rewashed. KDA_6_20215A3 - --Half inch gap at exterior door in receiving area where door meets floor. Door of kitchen open leading into receiving area at time of inspection. KDA_6_30112 - --Paper towel dispenser at warewashing area handsink not working. PIC tried to get maintenance to fix dispenser. NOT COS. KDA_6_30114 - --No handwashing signage at handsink in warewashing area, or at handsink in middle of kitchen. COS supplied KDA_7_10211 - --Squeeze bottle of thick pink substance in dish area with no label on bottle. PIC stated it was pot and pan detergent. COS labeled. KDA_7_20111A - --In dry storage area, spray can of stainless steel cleaner stored on shelf next to box of gloves and a stack of coffee filters. KDA_8_30411A - --Current license not posted. License that expired in 2014 is posted on front wall of kitchen.
