  • KDA_5_20515B - --The faucet on the right side of the 3 compartment sink does not shut off when the water is turned off. The PIC had to shut off the water valve. (Note: it was called to be serviced).
  • KDA_6_20211A - --There is one light fixture near the back door that does not have a protective cover.
  • KDA_6_50111 - --There are 2 small holes on the wall behind the stove. The wall at the mop sink is broken and exposed. The wall below the dish wash line is broken and exposed. There are broken floor tiles below the dish wash line.

  • KDA_3_30211A1b - --In the True RIC there was 2 gallons of milk being stored directly next to 2 open cartons of raw hard shell eggs (milk carton was leaning on the eggs). COS-rearranged items.
  • KDA_3_30212 - --On the wooden counter there was a green plastic bowl with a white powder substance and not labeled. The cook said it was powdered sugar.
  • KDA_3_50114A2 - --In the refrigerator downstairs, there was a large plastic food grade container of pot pie at 48.6F. It was made on 8/31/15, there was condensation on the lid and closed tightly (not able to cool down within 6 hours). The ambient air temperature of the unit is reading 35.1F. COS-discarded.
  • KDA_3_50116A2 - --In the McCall RIC there was a plastic food grade container of black beans at 44.0F and made on 8/28/15. There was also a plastic container of sour cream at 44.5F. The doors to the cooler have been opened and closed due to preparing the salad bar (less than 4 hours). The ambient air temperature of the unit is reading 29.7F. COS-discarded the black beans and kept the doors closed.
  • KDA_3_50118A2 - --In the True RIC there was an open bag of pepperoni slices without a date. The cook said they were opened on 8/28/15. COS-item was date marked.
  • KDA_4_20211A2 - --In a drawer across from the stove there were 3 rubber spatulas with cracked edges. COS-discarded. On the side of the ice machine there were 2 plastic ice scoops, one with a broken edge and another one had a crack on the edge. COS-discarded.
  • KDA_4_60111A - --The large metal Hobart mixer had dried food debris on the metal guard and on the metal insert where the whisk is attached. It has not been used recently. There was one (22qt) plastic food grade container with the sticker left on the side and stored clean. In a drawer across from the stove there was one metal spatula with dried food debris and stored clean. COS-the cook cleaned the mixer and placed the other items in the ware wash area.
  • KDA_4_60212B - --The microwave in the kitchen had dried food debris on the ceiling and not used recently. COS-cleaned.
  • KDA_5_20511B - --There was a knife being stored in the handwashing sink. The cook said he uses the knife and stores it in the sink to use later. COS-removed the knife.
  • KDA_5_20515B - --The hot water and cold water handles leak when water is supplied at the 3 compartment sink.
