  • KDA_5_20411A - --There is no hand washing sink. Employee said he washes his hands in the restroom. Inspector asked about variance, person in charge says he does not know if they have one.
  • KDA_5_501113 - --The outdoor trash dumpster lids were not closed.
  • KDA_6_20211A - --In the walk in cooler, there is one light set that is not shielded.
  • KDA_6_20214 - --The unisex restroom door is not self closing. Person in charge says its a safety issue, since they are open 24 hours.
  • KDA_6_50111 - --In the walk in cooler, there is some floor tile that is missing. One ceiling tile is missing in south east corner above the reach in cooler. One stained ceiling tile in the south west corner above the fountain pop area. There is missing baseboard in the corner by the door to the walk in cooler, there is a build up of debris from the missing baseboard in that corner. The toilet tank is missing off the toilet.

In the walk in cooler, there is trash, single serve items under the shelves and on the floor, several empty boxes are stored on the floor. There is also a trash can and a shelf that are both over flowing with outdated and adulterated items that are sitting in the walk in cooler.
