- KDA_3_50116A2 - --Commercial potato salad at 47F in reach in cooler (dorm/residential size). Opened can of baked beans on shelf of pantry at 58F. PIC stated it was opened for lunch. COS moved both items to freezer.

- KDA_4_70211 - --Scoop for potato salad rinsed in 3 vat sink and used in potato salad. Scoop was not washed, rinsed, and sanitized. COS education

- KDA_5_20511A - --Handsink blocked by food tray with a bucket of pickles on top of tray. Handsink was inaccessible, so PIC washed her hands in the 3 vat sink instead of the handsink. COS education, unblocked.
