In the meat department WIC, raw ground turkey was stored above RTE ham. COS Ground turkey was removed. In the meat department WIC, raw bacon was on a rack stored above RTE ham steak. COS Bacon was removed. In the bakery WIC, raw shell eggs were stored on a rack above RTE cheese. In the produce storage area, a bottle of vegetable wash was stored above banana peppers, and minced garlic. In the produce WIC, raw shell eggs were stored on a rack above RTE condiments.
- KDA_3_30212 - --In the meat department, a bottle of clear liquid was stored under a prep table without a label. PIC said it was water. COS PIC labeled the bottle.
- KDA_3_30511A1 - --In the seafood room, a bag of styrofoam was stored on the floor next to the ice machine. COS PIC put bag on crate, off the floor. In the bakery, a bag of flour was stored on the floor. In the bakery freezer, a boxes of cookie dough was stored on the floor. In the bakery, single service food containers was stored on the floor. In the Deli, box of single service cups were stored on the floor in front of the handwashing sink near cash register. COS box was removed. In the deli/kitchen, a box of hyvee cups were stored on the floor. COS Box was removed.
- KDA_3_50116A1 - --Baked chicken was being held as hot had a measured temperature of 119F and 127F in the deli kitchen area display warmer. COS Chicken was put back in oven for rapid heating.
- KDA_3_50116A2 - --In the deli area at the sandwich station, sliced tomatoes had a measured temperature of 52F. PIC said the package was open at 6am. COS Tomatoes were discarded.
- KDA_3_50118A2 - --In the bakery WIC, a milk container did not have a open or discard date. PIC said it was opened 10/13 at 10am.
- KDA_4_20211A2 - --In the meat department WIC, RTE breaded stuffing and beef was stored in cracked containers with cracked lids. In the produce WIC, RTE cut honey dew, cauliflower, cut pineapples, cut cantaloupe, and cut celery was stored in cracked containers with cracked lids.
- KDA_4_60111A - --In the meat department, inside of the meat grinder bin was dried food debris. Dried food debris was also on the large mixing arms. The meat grinder was stored as clean. In the bakery stored as clean, dough cutters and pizza cutter, other utensils were in a drawer had dried food on the surface. COS Utensils were taken out of drawer to be washed, rinsed, and sanitized. In the bakery in the back storage area, stored as clean burnt pans were stacked and had dried food debris on the surface.
- KDA_4_60111B - --In the bakery, bread pans had a build up of grease.
- KDA_4_60111C - --In the bakery, the top of the large mixers had a build up of dust and flour. In the bakery WIC, there was dried food debris on the walls. In the bakery, behind the racks stored as clean was dried food debris on the walls. Fan covers in the temperature controlled room in the produce area had a build up of dust.
- KDA_4_60212B - --In the bakery, the microwave had dried food and food debris on all 3 walls, the ceiling, and the bottle surface.
- KDA_5_20511B - --A rag was stored inside a handwashing sink basin in the bakery.
- KDA_6_501111A - --In the bakery, a fly landed on RTE cookies that were being decorated. COS Cookies were discarded. In the bakery, 50 to 60 drain flies were flying or on clean pans, wall, ceiling, and a rack.
- KDA_6_50112A - --In the bakery, deli/kitchen area, and the meat department had food debris on the floor and on the walls.
