KDA_3_10111 - --Cooked sausages were in a plastic storage container without date marking. PIC did not know when the sausages were place or opened. COS, sausages were discarded by the PIC. Mold strawberries were in the walk in cooler. COS, strawberries were discarded.

KDA_3_50118A2 - --An open package of fully cooked ham was in the walk I cooler without an opened package date. PIC stated the ham was opened on 8-8-15. COS, ham dated.
KDA_3_50119A1a - --Cube cut ham 60F was in a food storage without any of cold holding. PIC stated the ham will be discard after 4 hours. PIC did not have a written procedure for using time as a public health control.
KDA_3_50119B2 - --Cube cut ham 60F was not marked with any identified time. COS,PIC placed identified time on the ham.
KDA_4_90311B - --Food storage containers were stack wet and stored as clean on a storage rack.
KDA_6_501112 - --A dead roach was on the floor in the food storage area. PIC provided a pest control invoice with a service date of 8-7-15.
KDA_7_20111A - --A bucket filled with sanitizer was stored next to vegetable oil on a shelf under a prep table. COS, cooking oil removed.
KDA_7_20111B - --A can of dust cleaner was stored above single-serve cups that were in a cabinet space in the waitress/waiter prep area.
KDA_Defacto - --Hash brown potatoes 170F,eggs 192F. --Hot water 144F handsink. --RIC boiled eggs 42F RIC milk 42F WIC cut lettuce 41F WIC ham 41F. Advise the PIC the RIC, WIC units needed to watch or service due to the temps were barely acceptable.
