- KDA_3_30412B - --In storage area, a cup with no handle was being used as a scoop in a container of granola powder. COS: Educated, employee discarded cup.
- KDA_3_50116A2 - --On serving table, salads were held at 50.2F and cut melons were held at 45.6F. In cafeteria, real butter was held at 51.4F. Person in charge (PIC) said items had been removed from refrigeration approximately thirty minutes earlier. COS: Items were served immediately. Unused items were returned to WIC for rapid cooling.
- KDA_3_50118A2 - --Milk and buttermilk in the WIC were not labeled with the date they were opened. PIC said they were opened 1/21/16. COS: Employee correctly date labeled items.
- KDA_4_60111A - --In storage area, a container in use was soiled with sticker residue. In dish area, a large metal container was soiled with food debris. COS: Items moved to warewashing for additional cleaning.
- KDA_6_50112A - --There is dust, dirt, and food debris on floors behind ice machine and under shelving in storage area.
- KDA_Defacto - --All potentially hazardous items in freezers are frozen. --From oven: steak fingers - 175.4F. --Items thawing under refrigeration were marked only with the date they were frozen. PIC said items had been removed from the freezer yesterday (within twenty-four hours). Discussed updating dates as soon as items are removed from the freezer. Employees were aware to count date of freezing as day 1. --Items thawing under refrigeration. --Midwest Pest Control, no paperwork available. --On steam table: steak fingers - 154.7F
